Step 4. Print, read, sign (original signatures required), date, and return all appropriate forms to us. Because original signatures are required on these documents, you will not be able to fax or email them to us. Sorry! Mail (we recommend priority mail) or 2nd day or overnight courier will be necessary. Please give us a call at 800 634-9650 if you have any questions. We'll be happy to help you fill these out over the phone.
